How to Structure a Nursing Care Plan Using the Nursing Process

A step-by-step guide to the nursing process (ADPIE), writing nursing diagnoses, setting SMART goals and justifying interventions with evidence.

Care plans are one of the most common assessments in nursing education because they show how you think clinically. Markers look for a logical chain from patient data to measurable goals to justified interventions.

The nursing process: ADPIE

  1. Assessment — gather subjective and objective data about the patient.
  2. Diagnosis — identify the patient's actual or potential health problems.
  3. Planning — set patient-centred goals and expected outcomes.
  4. Implementation — carry out evidence-based nursing interventions.
  5. Evaluation — measure whether goals were met and revise the plan.

Writing a nursing diagnosis

Many programmes use the NANDA International format: the problem, related to its cause, as evidenced by the signs and symptoms you assessed. For example: Acute pain related to surgical incision as evidenced by a self-reported pain score of 7/10 and guarding of the abdomen.

Setting SMART goals

Goals should be Specific, Measurable, Achievable, Relevant and Time-bound, and written from the patient's perspective. “Patient will report pain of 3/10 or less within one hour of receiving analgesia” is measurable; “patient will feel better” is not.

Justifying interventions

Each intervention needs a rationale explaining why it works, supported by current evidence such as clinical guidelines or peer-reviewed research. This is where many care plans lose marks: an intervention without a referenced rationale is only a task list.

Evaluation closes the loop

State whether each goal was met, partially met or not met, based on the outcome data, and explain how you would modify the plan. Evaluation shows that care is continuous, not a one-off checklist.
